Pool Reservation Form

The site rotation schedule indicates that your institution is eligible to host a league tournament this coming season. To prevent any misunderstandings, you must complete this form and return it to the office. You may complete it on line and submit it directly.

If this date presents a major conflict with another school event that will make securing hotels difficult (football game, parent’s weekend, homecoming, etc.), list that information in the space provided.

By signing below, you agree to the following: (if not submitted online, please sign and either fax to 610-277-7382 or mail to CWPA office).

  • The institution will bear all facility costs associated with hosting (pool rental, etc.)
  • The pool will be available for the hours listed above unless alternatives are provided.
  • The team agrees to provide trained desk officials to work all games (may be students).
  • The pool will be set up to meet the size requirements stated above.
  • Host team must submit scores immediately following the games (Sunday evening), accompanied by a written summary of the competition to be posted on the web site.
